اللَّهُمَّ اغْفِرْ لِفُلَانٍ وَارْفَعْ دَرَجَتَهُ فِي الْمَهْدِيِّينَ
Allahummaghfir li-fulanin warfa' darajatahu fil-mahdiyyin
O Allah, forgive [him/her] and elevate his/her station among the guided.
